A Serious Case Review or Part 8 Review is conducted by the Local Safeguarding Children Board when a child dies and Abuse or Neglect are known to be a factor in the death or where a child sustains a potentially life-threatening injury or serious and permanent impairment of health and development. 

The purpose of the Review is to establish whether there are lessons to be learned from the case, identify what those lessons are and how they will be acted upon and improve inter-agency working as a result.
